Comments:
An unannounced monitoring inspection was conducted 4/3/2023 from 11:05am through 12:00pm. There were 38 children directly supervised by 6 staff. The physical plant, programming, menu, 5 staff records, 5 children?s records, 5 medications, emergency drills, emergency supplies, and policies were inspected. Children were observed playing a freeze dance game and eating lunch. Positive interactions between children and staff were observed during this inspection. There was an adequate number of CPR/First Aid, Daily Health Observation, and MAT trained staff on site at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-780-130-A
Description: Based on record review, the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Evidence: Child #3 and Child #5 were both present at the time of inspection and did not have documentation of immunizations on site.

Standard #: 8VAC20-780-60-A-8
Description: Based on record review, the center did not obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence: Child #7 has a diagnosed food allergy and didn't have a written allergy care plan on file signed by a physician.

Standard #: 8VAC20-780-510-B
Description: Based on medication and documentation review, the center did not ensure that they obtained written authorization from the parent for all medications on site.

Evidence: Two emergency medications were on site for Child #8 on 4/3/2023. There was no written authorization from the parent on site.
