Comments:
An unannounced monitoring inspection was conducted on 04/04/2023 and concluded with an electronic record review on 04/13/2023. There were 15 children, ages 3 years- 5 years, and 3 staff providing direct supervision present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The children were observed in mealtime, teacher directed activities, and departure for field trip. The inspector discussed the following with provider: allergy list. A total of 5 children?s records, and 3 staff records were reviewed. The inspector arrived for the inspection at 9:00 am and departed at 10:00 am.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on record review, the center failed to ensure that the separate record for each enrolled child contains all of the elements as required by the standards.
Evidence:
1. The record for Child 1 did not contain the following information: work phone number for each parent (an employer was listed but the record did not contain a work phone number).
2. The record for Child 2 did not contain the following information: name address and phone number of two designated people to call in an emergency if a parent cannot be reached (the addresses for the second contact was incomplete).

Plan of Correction: Staff will obtain information from parents.

Standard #: 8VAC20-780-540-E
Description: Based on observation, the center failed to ensure the following non-medical emergency supplies shall be required: one working battery-operated radio in each building used by children.
Evidence: During the inspection, a working battery-operated radio was not available.

Plan of Correction: Staff will obtain batteries.
