An unannounced monitoring inspection was conducted on 3/29/2023 at 12:35pm. There were 40 (infants to 5yrs) directly supervised by 5 staff. The physical plant, 4 staff records, 5 children?s records, children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed preparing for and taking naps. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Standard #: 8VAC20-780-350-B-1 Description: Based on observation, the facility did not ensure that ratios of 1 staff to 4 children was maintained in the birth to 16 month classroom.
Evidence: observed 5 infants and 1 staff member in the infant classroom.Plan of Correction: The second teacher in the Infant classroom was out sick with a doctor's note stating she was not allowed to return to work until 3/30/2023 because she was contagious.
Standard #: 8VAC20-780-350-B-2 Description: Based on observation, the facility did not ensure the ratio of 1 staff and 5 chidlren in the 16 months to 24 months classroom.
Evidence: observed in the toddler classroom (16m to 24m) 1 staff with 7 children.Plan of Correction: The second teacher in the toddler classroom was out sick with a doctor's note stating she was not allowed to return to work until 3/31/2023 because she was contagious.
Standard #: 8VAC20-780-350-B-4 Description: Based on observation, the facility did not ensure the ratio of 1 staff to 10 children for 3yrs to school-age.
Evidence: observed that the Pre-K classroom, 3-4 years, was 1 staff to 11 children.Plan of Correction: A potential second teacher for the pre-k classroom was waiting for Background check results as well as a TB test before starting work.
All results have been received and a second teacher was hired 2 days after inspection.
