Technical Assistance:

Comments:
A monitoring inspection was conducted on 04/11/2023 between the hours of approximately 11:10 am and 2:50 pm. There were 99 children present with 21 staff supervising.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-130-A
Description: Based on record review, the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
1) The record for Child #2 (start date 01/17/2023) did not contain an immunization report.

Plan of Correction: Parents have been notified and given an ultimatum. Child will not attend until immunization report is obtained.

Standard #: 8VAC20-780-130-E
Description: Based on record review, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
1) The most recent immunization report in the record for Child #3 (dob 03/09/2022) was dated 07/12/2022.

Plan of Correction: Parents were contacted and documents will be faxed in.

Standard #: 8VAC20-780-140-A
Description: Based on record review, the center did not ensure that each child have a physical examination by or under the direction of a physician: before the child's attendance; or within 30 days after the first day of attendance.
Evidence:
1) On the date of inspection, the record for Child #2 (start date 01/17/2023) did not contain the results of a physical examination.

Plan of Correction: Parents have been notified and given an ultimatum. Child will not attend school until physical obtained.

Standard #: 8VAC20-780-160-A
Description: Based on record review, the center did not ensure that documentation of negative tuberculosis (Tb) screening for each employee met all requirements.
Evidence:
1) The documentation shall have been completed within the last 30 calendar days of the date of employment. - The Tb screening form in Staff #2's (start date 03/06/2023) record was dated 1/07/2023.
2) The documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children - The Tb screening form in Staff #1's (start date 01/23/2023) record was dated 03/01/2023.

Plan of Correction: Both employees went for screening on 04/11/2023 and awaiting the results.

Standard #: 8VAC20-780-40-K
Description: Based on review of the center's policies and interview with the center's director, the center did not develop written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness.
Evidence:
1) On the date of inspection, the center's director stated that the center had not developed written procedures for prevention of shaken baby syndrome or abusive head trauma.

Plan of Correction: Written procedures have been added to handbook.

Standard #: 8VAC20-780-240-C
Description: Based on record review, staff's orientation training did not include all required topics.
Evidence:
1) The orientation training log in the records for Staff #1 and Staff #2 did not contain documentation that staff had been trained in prevention of shaken baby syndrome and abusive head trauma, including procedures to cope with crying babies or distraught children.

Plan of Correction: Staff #1 + 2 are working on it.

Standard #: 8VAC20-780-240-E
Description: Based on record review, the center did not ensure that staff completed orientation training in first aid and cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care, within 30 days of the first day of employment.
Evidence:
1) The record for Staff #1 (start date 01/23/2023) and Staff #2 (start date 03/06/2023) did not contain documentation of having completed orientation training in first aid and CPR.

Plan of Correction: Both staff signed up for an online course.

Standard #: 8VAC20-780-240-I
Description: Based on record review and interview with staff, documentation of orientation training was not be kept by the center as part of the staff member's record.
Evidence:
1) The center's director stated that Staff #4 completed orientation training upon hire on 10/17/2014, however, documentation of Staff #4's orientation training was not located in their record.

Plan of Correction: Document was located. It was misplaced.

Standard #: 8VAC20-780-550-F
Description: Based on documentation review, emergency lockdown procedures were not practiced at least annually.
Evidence:
1) On the date of inspection, the 2022 emergency drills log provided for review, did not have a lockdown drill documented.

Plan of Correction: The lockdown drill was conducted in 2022 but document misplaced during the move.
