Comments:
An unannounced monitoring inspection was conducted beginning at 11:20 a.m. and ending at 4:30 p.m. with the executive director. There were 64 children in care ages 13 months through 5 years old under the care and supervision of 15 staff. Child care areas, supplies, materials and playground areas were observed. Rest time was observed.

Five children's records and five staff records were reviewed. Seven medications were reviewed. Emergency drills were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation, not all areas and equipment of the center were maintained in safe, operable condition.
Evidence:
1. On the Pavilion playground area there is a compost area that has three steel T-post fence posts that are sticking out of the compost pile that create an impalement hazard.
2. On the Pavilion playground, there are five steel T-post fence posts leaning against a tree with sharp protrusions at the bottom of the post. One of the posts was visibly rusted. The t-posts create a variety of hazards for falling/head/body, cutting/skin snagging, and impalement.
3. On the Pavilion playground, a slackline obstacle course type climber is installed between trees by way of ratchet strap/tie down straps. There is no verification of the safety of installation for use of preschool or school age children.
4. In the garden area, there were four steel rebar posts used to secure plastic planters on three concrete edges and one rebar was completely exposed. The rebar creates impalement and snag/cutting hazards.

Description: Based on observation, it was determined that resilient surfacing for playground equipment did not meet requirements of at least six inches in depth under and around in use/fall zones. Evidence: 1. There was visible dirt under the swings. In the use/fall zones in front of the swings there was visible dirt. There was not at least six inches of pea gravel in use/fall zones for the swingsets. 2. There is no resilient surfacing for the obstacle course that was installed between two trees.

Description: Based on observation, climbing equipment on the playground used by preschoolers was more than six feet high. Evidence: On the Pavilion play area, there is a climbing obstacle course installed on two trees that was more than six feet high.

Description: Based on observation, it was determined that the center failed to ensure that nap mats were equipped with a bottom linen as required. Evidence: In the toddler room, seven children were observed resting on nap mats and had no sheet or bottom linen. In the two year old room, twelve children were observed resting on nap mats and had no sheet or bottom linen.

Description: Based on observation and interview, it was determined that the center failed to ensure that pillows were assigned for individual use and covered with pillow cases. Evidence: In the pavilion classroom of preschool children, there were approximately three pillows in use during nap that were not assigned for individual use or covered with a pillow case.

Description: Based on review of medication, the center failed to ensure that their medication policy was followed. Evidence: Child #10, had children?s motrin on site, signed authorization only from parent on 10/22/2022 for 10 days. There was no long term authorization for the medication.
