Technical Assistance:
1. Recommend an internal practice of knowing when medications expire and when they must discarded be implemented.
2. Discussed minor injuries of children to include whenever first aid is rendered. The inspector will send the regulation and a model injury report form.

Comments:
Conducted an unannounced monitoring inspection at 12:19pm. Observed 21 children + 3 direct-care staff in the extended-day program. Ratios and supervision were in compliance at the time of inspection. Children were observed having active indoor play in the gym. The program areas were found to be clean, safeguarded appropriately, and sufficiently supplied with equipment for the children. There were areas of non-compliance with standards and laws reviewed. Questions about this inspection may be directed to pamela.sneed@doe.virginia.gov

Standard #: 8VAC20-780-510-P
Description: Based on observations made, it was determined that an expired medication was not picked up by the parent within 14 days or disposed of. Evidence: A medication belonging to Child #3 expired in February 2023 was found to be in the extended day medication box.

Plan of Correction: Injury reports for minor injuries of children will be written and documented.

Standard #: 8VAC20-780-540-E
Description: Based on observations made it was determined that a required non-medical emergency supply item was not on-site. Evidence: There was not a working battery-operated radio. There were no batteries for the radio on-site.

Plan of Correction: Batteries will be installed for the radio on-site.

Standard #: 8VAC20-780-550-P
Description: Based on interview conducted, it was determined that written records of children?s serious and minor injuries have not been maintained. Evidence: The program director reported that injury reports for minor injuries of children have not been written within the extended day program.

Plan of Correction: The parent will be notified to provide a new medication for Extended Day.
