Comments:

An unannounced monitoring inspection was conducted on 4/5/23 from 3:50pm to 4:15pm with program staff. There were seven children in care, supervised by two staff. Children were observed cleaning up and transitioning outside to the playground. Required postings were observed. The attendance, allergy list, and emergency drill log were reviewed. Two child records were reviewed. A review of required on-site documentation was conducted for staff certified in CPR/First Aid, Daily Health Observation Training, and Medication Administration Training (MAT). Off-site staff records were reviewed at the Ron Rosner office on 3/23/23. Staff reported there was no medication on-site but the center has staff with current certification in Allergy & Anaphylaxis Medication Administration Training (AMAT). Description:	Based on review of three staff records, the program did not obtain the results of a search of the central registry for one staff within 30 days of employment. Evidence: There were no central registry results on record for Staff #3, with an employment date of 8/8/22.
Plan of Correction:	Have the central registration within 30 days of hire.
Standard #:	8VAC20-780-70
Description:	Based on review of three staff records, the program did not obtain all required documentation for staff records. Evidence: 1. On 4/5/23, the required on-site documentation for Staff #3, to include the name, address, verification of age, job title, and date of employment; and name, address, and telephone number of a person to be notified in an emergency, was not on-site for Staff #3. 2. The off-site record for Staff #3 did not include documentation of a second reference check, as to character and reputation as well as competency, prior to employment. 3. The record for Staff #2 did not contain documentation to demonstrate that the individual possesses the education or certification required by the job position.
Plan of Correction:	References will be completed before hire.
Standard #:	8VAC20-780-240-A
Description:	Based on review of three staff records, the program did not ensure that the Virginia Department of Education-sponsored orientation course was completed by one staff within 90 calendar days of employment. Evidence: The VDOE-sponsored orientation course certificate on record for Staff #3, with an employment date of 8/8/22, was dated 4/17/23.
Plan of Correction:	Be sure to have staff complete training in the designated amount of time.
