Comments:
A subsidy health and safety inspection (SHSI) was conducted on March 30, 2023. The inspector arrived at 3:15PM and departed at approximately 4:30PM. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental Inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the site director at the exit interview.

Standard #: 8VAC20-790-600-B
Description: Based on record review and interview, it was determined that the vendor did not ensure that within the first 90 days of employment the vendor or designee shall complete the Virginia Preservice Training for Child Care Staff.
Evidence:
1. Staff #1 (date of hire 10/15/21) did not have documentation of the completion of the Virginia Preservice Training for Child Care Staff.
2. Staff #3 confirmed that there was no documentation to Staff #1?s file and unable to confirm the training was taken.

Description: Based on record review and interview, the vendor did not ensure that all staff have current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care.

Evidence:
1. The record for Staff #2 does not contain documentation of current certification in CPR. The last certification for Staff #2 expired on January 5, 2022.
2. Staff #3 confirmed that the record for Staff #2 lacks documentation of current certification in CPR and only has the online component of training completed. Staff #3 does not have the full certification for CPR.
