Comments:

A monitoring inspection was initiated on this date and concluded. There were 50 children present, ranging in ages from 5 years to age 11, with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 4 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #:	8VAC20-780-70
Description:	Based on a record review and staff interviews, the licensee did not ensure to obtain all required items for staff records. 1. Staff #2 did not have documentation to demonstrate they have the training/education and experience required for program lead. Staff #5 said Staff #2 is a designated as program leads. 2. There was not emergency contact information and required health related information for Staff #3 at the center
Plan of Correction:	Area manager and program manger will work together to ensure proper documentation iso n file for program leads as well as all emergency forms.
Standard #:	8VAC20-780-510-L
Description:	Based on observation and staff interviews, the licensee did not ensure medication, except for those prescriptions designated otherwise by written physician's order, including refrigerated medication and staff's personal medication, shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: There were two medications (Albuterol and pain reliever ) observed on a shelf in the unlocked office which children had access to.
Plan of Correction:	Medication was put in required lock box on night of 4-8-23. Program manager will ensure all medication is put into the lock box once medication is provided by the parent.

Description:	Based on observation and staff interviews, when centers choose to provide meals or snacks, there shall be a menu listing foods to be served for meals and snacks during the current one-week period that is dated and posted in a location conspicuous to parents or given to parents. Evidence: Staff #1 confirmed there was not a menu available or given to parents.
Plan of Correction:	We use a shared space where an event is being held. menu was posted again the morning of 4-19-2023.,

Description:	Based on observation and staff interviews, the licensee did not ensure documents related to the terms of the license are required to be posted on the premises of each facility. These include the most recently issued license. Evidence: There was not a license posted.
Plan of Correction:	We use a shared space where a event was happening. Licensing board was put back out next morning 4-19-23

Description:	Based on observation and staff interviews, the licensee did not ensure the license and any other documents required by the Superintendent were posted in a conspicuous place on the licensed premises. Evidence: The most recent inspection results dated September 27, 2022 were not posted in a conspicuous place.
