Comments:
An unannounced on-site renewal inspection was conducted on 04/12/2023. There were 36 children, ages 11 months-12 years, and 6 staff members present during the inspection. The inspectors observed the following: free choice activities, large motor play outside, and lunch. A total of 5 children?s records and 5 staff records, were reviewed. The inspector discussed the following with staff: attendance requirements, playground safety. Additional information was requested and was sent 04/24/2023. The inspectors arrived at 9AM and departed at 11:30 AM.

Description: Based on review of eight children's records, the center failed to ensure that all children had a physical examination within 30 days of enrollment.

Evidence:

1. The record for child # 5 had a documented start date of 8/8/2022 and was present on the day of the inspection did not have a physical examination.

Based on review of staff records, the center failed to ensure that all records were complete per the standard.

Evidence:

1.Staff # 4 had a documented employment date of 8/22/2022 and did not have one of the two required reference checks prior to employment. Staff was observed working with a grouping of children.

center was maintained in a clean, safe and operable condition.

Evidence:
1.The outside plastic storage container that stood approximately 6 foot high, had a door of the hinges. This posed pinch hazard to the children.

Evidence:

1.The cabinet in the infant room there was a bottle of Tylenol that was unlocked. The Tylenol was placed in a basket was on a high shelf and was not accessible to the infants in care.

Evidence:
1.Staff # 1 verified that in the Bright Wheel system they indicate that a medication was
given to Child # 5. The following is not noted in the system when medication is administered: the amount of medication, the type of medication, the staff member administering the medication, any adverse reactions and medications errors are not noted in the system.
2. Verification from requested documents from staff # 6 that the medication was given to child # 5 on the following dates and times: 03/06/2023 at 11:17 AM and 4:10 PM, 03/07/2023 at 10:21 AM, 03/08/2023 at 2:48 PM, 03/10/2023 at 2:38 PM and 03/30/2023 at 1:55 PM.

Description: Based on review of medications and authorizations, the center failed to ensure that all medications were returned to the parent or guardian within 14 days after the authorization expired or disposed of by the center.

Evidence:

1.Medication authorization for Child # 5 expired 14 days after 02/22/2023. As of 04/12/2023, the medication had not been returned to the parent or disposed of by the center.

Description: Based on observation of the nonprescription drugs, the center failed to ensure that all nonprescription drugs are used in accordance with the manufacturer?s recommendations.

Evidence:

Child # 5 that was 11 months old had non-prescription Tylenol at the center and 2.5 ML was administered to the child on 03/06/2023 at 11:17 AM and 4:10 PM, 03/07/2023 at 10:21 AM, 03/08/2023 at 2:48 PM, 03/10/2023 at 2:38 PM and 03/30/2023 at 1:55 PM. The manufacturer?s recommendation is for children under the age of 2 years to consult a physician to administer.

Description: Based on observation of the infant room, the center failed to ensure that all infants using a feeding table, or a highchair had the protective belt fastened securely.

Evidence:

There was an infant that was 11-month-old, and one that was 13 month old sitting in the feeding table without the safety belt securely fastened.
