Comments:
An unannounced monitoring inspection was conducted on April 25, 2023, to determine the center?s compliance with licensing standards. The inspection began at approximately 9:20 AM and concluded at 12:00 PM. There were 145 children present, ranging in ages from 4 months to 6 years, with 20 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies, transportation, and nutrition. A total of 10 child records and 10 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Description: Based on observation and interviews, the center did not ensure to maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions. This list shall be dated and kept confidential in each room or area where children are present.
Evidence:
1. In the Flamingo?s classroom there were red octagon allergy tags with the child?s name and allergy visible and not kept confidential.
2. In the Infant II classroom, there were red octagon allergy tags with the child?s name and allergy visible and not kept confidential.

Standard #: 8VAC20-780-270-A
Description: Based on observation and staff interviews, the licensee did not ensure the areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Unsafe conditions include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.
Evidence:
1. The red chair in the older 3 and 4?s classroom fabric was torn (approximately 3 1/2 inches long) and the inside filling/ material was exposed.
2. Two red poles at the corner of the play equipment on the 2nd playground has broken edges. The broken edges create a scratching hazard.
