Comments:
A monitoring inspection was conducted on 04/21/2023 from 8 am to 1:50 pm. There were 63 children present, ranging in ages from three months to five years, with 13 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of six child records and 13 staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Please complete the plan of correction and date to be corrected sections for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s): 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Description: Based on the review of 13 staff records, the program failed to obtain a copy of the results of a criminal history record check from any state in which an individual has resided in the preceding five years for one staff.

Evidence
The first day of employment for Staff #6 is 11/17/2022. There were no results from a criminal history record check for a previous state of residence.

Description: Based on the review of 13 staff records, the program failed to ensure that they have obtained the results of a central registry check within 30 days of employment for four staff.

Standard #: 8VAC20-780-500-B
Description: Based on observation, the program did not ensure that disposable diapers are disposed in a storage system that is either food operated or in such a way that neither the staff member's hand nor the soiled diaper touches the exterior surface of the storage system during disposal.

Evidence
1. On 04/21/2023 while observing toileting procedures in the Early 2s classroom, Staff #14 was observed lifting the lid of the disposal system with her hand and disposing of a soiled diaper instead of utilizing the foot operating mechanism.
