Comments:
A Subsidy Health and Safety Inspection was initiated on Wednesday, March 29, 2023 and concluded on Thursday, March 30, 2023. On March 29, the inspector was on site from 4:00pm to approximately 5:15pm. There were a total of nine children in care in the direct care of two staff members. During the inspection, the children and staff were observed participating in a variety of activities. Staff were observed having positive interactions with the children. All areas of the facility used by the children were inspected. The center is equipped with toys and supplies and items were available to the children. The required postings were reviewed and found to be in compliance. Medication is administered, but there are no medications on-site at this time. During the inspection, five children's records were reviewed and applicable staff information required to be at the center was reviewed. On March 30, the inspector conducted an unannounced inspection at the administrative office to review two staff records.

A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-790-600-D-1
Description: Based on a review of two staff records, the vendor did not ensure one staff member had current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.

Evidence: The CPR certification in the record of Staff #1 expired on 11/14/2022.

Plan of Correction: Attended course on 04/28/23.

Standard #: 8VAC20-790-600-D-2
Description: Based on a review of two staff records, the vendor did not ensure one staff member had current certification in first aid appropriate to the ages of children in care.

Evidence: The first aid certification in the record of Staff #1 expired on 11/14/2022.

Plan of Correction: Attended course on 04/28/23.

Standard #: 8VAC20-790-600-F
Description: Based on a review of two staff records and interview, the vendor did not ensure two staff members who work directly with children completed the department's health and safety update course annually.

Evidence: 1) The record of Staff #1 (DOE: 06/07/21) and the record of Staff #2 (DOE: 06/14/21) did not contain the department's health and safety update course. 2) During interview, a member of management confirmed Staff #1 and Staff #2 have not completed the training. The center was approved as a vendor on 03/28/22, therefore, the annual training should have been completed no later than 03/28/23.

Plan of Correction: All staff will be required to take the health and safety update training as soon as possible and every year thereafter.
