Technical Assistance:
*Discussed with the director that she needed to add age verification to the staff records. She does have staff verification on site for review, but it is locked and staff do not have access to the cabinet when the director is not on site, resulting in the inspector not being able to review it during the inspection visit.
*Discussed that the facility's evacuation drill log was incomplete. The director stated that she had been using the same log for the last couple of years with no issue. I discussed with the director that the drill log needed to have all the required documentation, and to update their current log or use the one provided on the VDOE website. The inspector observed the following missing from the emergency evacuation plan log: 1) person conducting drill; 2) method of notification; 3) any special conditions; 4) problems encountered; 5) weather conditions.

Comments:
An unannounced monitoring inspection was conducted on 4/24/2023 from 10:00am to 11:45am. There were 34 (Infants to preschool) directly supervised by 6 staff. The physical plant, 2 staff records, 5 children?s records, no children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, and playing outside. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Standard #: 8VAC20-780-350-B-1
Description: Based on observation, the facility did not ensure that the ratio for children in the infant classroom was maintained at 1 staff to 4 children.
Evidence: observed in the infant classroom five children and one staff member. The staff stated that the child was not in there long, and removed one of the children from the classroom.

Plan of Correction: There was an extra staff in the facility she was helping out and holding one child was calling for help to open a child gate. The area is an open layout accidently she told the infant teacher if she can hold the child till she opened the gate and went back and took the child which was really less than a minute. When this happen the inspector was leaving the facility when she observed this. The staff apologize about the mistake, and said it won't happen again. During the start of inspection there were 4 till this quick mistake happen.

Description: Based on observation, the facility did not ensure that medications are kept in a locked place.
Evidence: observed that the first aid kits were kept in a cabinet near the director's office that remained unlocked during the inspection visit. Observed in a first aid kit kept in the closet had several packets of aspirin inside.

Plan of Correction: Staff took out brand new first aid kit which we had extra never opened the stuff inside the kit was still sealed. I opened and trashed the aspirin and locked the closet. the one we always use was outside at the playground with the staff. We will make sure we remove the aspirin from the kits as soon as we purchase them.
