Technical Assistance:
The required annul preservice update training for all staff was discussed with the Program Director.
Clear documentation of employee hire dates is necessary to determine compliance with those Standards that are based around hire dates.
Although non-absorbent mats have been provided for nap time, they provide less than 1/4 inch of cushioning. Cushioned mats are required by the Standards for nap time.

Comments:
A renewal inspection was conducted on 05/02/2023. There were 7 children present with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 staff records were reviewed.


Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director during the exit interview.

Violations:
Standard #: 22.1-289-036-B-4
Description: Based upon review of staff records and staff interview, the facility has not ensured that the results of a search of the child abuse and neglect registry have been obtained from any state in which an applicant has resided in the past five years.
Evidence:
1. The record provided for staff 2 did not include the results of a central registry search from the state of Florida, where, according to the Sworn Disclosure Statement provided, the employee lived within the past five years.
2. Staff 1 verified that a central registry check from Florida had not been obtained for staff 2.

Plan of Correction: The facility responded with the following:
Results of a central registry search for the state of Florida will be immediately requested. In the future, out-of--state background checks will be obtained prior to the employ of any applicant who has lived in a state other than Virginia.

Standard #: 8VAC20-780-130-A
Description: Based upon review of children's records, the facility has not ensured that documentation that each child has received the immunizations required by the State Board of Health is obtained before the child can attend the center.
Evidence:
The record provided for child 1 indicated that child 1 began attending on 9/11/2022. Documentation of the required immunizations was not obtained.

Plan of Correction: The facility responded with the following:
All children's records will be checked to ensure that immunization documentation is in each record. In the future, no child will be allowed to attend until an immunization record is provided.

Standard #: 8VAC20-780-140-A
Description: Based upon review of children's records, the facility has not ensured that each child has a physical examination by or under the direction of a physician within 30 days after attending.
Evidence:
The record provided for child 1 indicates that child 1 began attending on 9/11/2022. Documentation of a physical examination were not obtained for child 1.

Plan of Correction: The facility responded with the following:
All children's records will be checked to ensure that results of physical examinations are included in each record. A physical examination will be obtained for child 1 and results placed in the record.

Description: Based upon review of staff records, the facility has not ensured that tuberculosis screenings of staff are obtained at least every two years
Evidence:
The most recent tuberculosis screening in the record provided for staff 1 is dated 7/13/2020..

Plan of Correction: The facility responded with the following:
repeat tuberculosis screenings will be obtained by all staff at least every two years. Staff 1 will obtain an update as soon as possible.

Description: Based upon review of staff records, the facility has not ensured that each staff record includes all required documentation.
Evidence:
1. The record provided for staff 2 did not include documentation of the staff date of employment
2. The record provided for staff 4 did not have documented the employee's job title..

Plan of Correction: The facility responded with the following:
The date of employment and job titles will be clearly written in all staff records.

Description: Based upon review of staff records, the facility has not ensured that the Va Dept. of Education-sponsored pre-service orientation course has been completed within 90 calendar days of employment.
Evidence:
The record provided for staff 4, employed on 9/4/2021, indicated that staff 4 did not obtain the required preservice orientation training until 3/14/2022.

Plan of Correction: The facility responded with the following:
The required preservice orientation training will be obtained by staff within 90 days after employment.

Standard #: 8VAC20-780-240-E
Description: Based upon review of staff records, the facility has not ensured that staff have completed orientation training in first aid and cardiopulmonary resuscitation (CPR) as appropriate to the age of the children in care in obtained within 30 days of the first day of employment.
Evidence:
The record provided for staff 4 indicated a date of employment of 9/4/2021. The record did not include documentation that staff 4 obtained an orientation training in first aid and CPR.

Plan of Correction: The facility responded with the following:
Staff 4 will be trained in basic first aid and CPR.

Standard #: 8VAC20-780-245-A
Description: Based upon review of staff records, the facility has not ensured that staff complete annually a minimum of 16 hours of training appropriate to the age of the children in care.
Evidence:
1. The record for staff 1, employed since 2018, included documentation of only 1.5 hours of training during 2022.
2. The record provided for staff 3, employed since 2021, did not include any documentation of training in 2022.
3. The record provided for staff 4, employed since 2021, included documentation of only 11.5 hours of training in 2022.

Plan of Correction: The facility responded with the following:
Staff have received training during staff meetings and on-line, however documentation has not been included in the records. Training will be conducted/obtained and documentation placed in each staff record. Staff will complete the 3 hour preservice training update annually.

Standard #: 8VAC20-780-560-F
Description: Based upon review of the posted menu, the facility has not ensured that they follow the most recent nutritional requirements of the child Care Food Program (USDA).
Evidence:
The posted menu for the current week indicated that only one food group component was served at snack time.

Plan of Correction: The facility responded with the following:
Children will be provided with at least two differing food groups for each snack. For instance: grain and fruit.
