Daily activities for school age children shall include an opportunity for large motor activities. Discussed the importance of taking children outside to meet this standard when weather is appropriate, rather than only using the indoor gymnasium.

Comments:
An unannounced monitoring inspection was conducted from 4:30 - 5:00 pm with staff. There were ten children, ranging from five years to ten years of age, present with two staff supervising. Children were observed eating snack, washing hands and going to the gym to play. Two onsite staff records and three onsite child records were reviewed at the time of the inspection along with the daily attendance record, emergency drill log, written list of food allergies, sensitivities and dietary restrictions, four medications and the required posted information. One staff member on site has current Medication Administration Training (MAT) certification. Two staff records were reviewed off site at the YMCA office on 5/4/2023.

Standard #: 8VAC20-780-40-M
Description: Based on review of documentation, and an interview with staff, the facility failed to maintain a current written list of all children's allergies, sensitivities and dietary restrictions that is dated, kept confidential, and in each room or area where children are present. Evidence: The list posted on the open storage room door was not confidential, and the list did not accompany the children to the gym.

Description: Based on review of two child records, not all records contained all required information. Evidence: the record for child #1, did not contain the name, address and phone number for two designated people to call in an emergency if a parent cannot be reached.

Description: Based on review of four medications and authorizations, it was determined that the facility failed to follow their medication procedures as required. Long term prescription drug use and over-the-counter medication may be allowed with written authorization from the child's physician and parent. Evidence: the record for child #3 did not contain written authorization from the parent or physician for two long term emergency medications observed on site.

Description: Based on review of documentation, the facility failed to ensure there were at least two staff members with current certification in First Aid and CPR present, on site, during the facility's hours of operation and whenever children are in care. Evidence: there were two staff on site supervising children, and staff #2 had First Aid and CPR certification that was dated: 2/15/21, therefore expired.
