Comments:
A mandated monitoring inspection was conducted on May 2, 2023 from 10:55 A.M.-1:30 P.M. There were 28 children present, ranging in ages from three to five, with four staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and four staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-770-60-C-2
Description: Based on a review of staff records, the center failed to ensure that a central registry finding was completed and on file within 30 days of employment.

Evidence:
1. Staff #1's date of employment was 02/07/2023. A central registry finding had been submitted on 02/08/2023. There was not a completed central registry finding or documentation of an update in the staff records.
2. Administration verified the date of employment, the date the central registry finding was submitted, and that there had been no follow-up completed.

Plan of Correction: Administration thought the check had been completed and did not realize they had not obtained the results. Administration reached out to the Office of Background Investigations and working to find out the status of the submitted report.

Standard #: 8VAC20-780-290-A-3
Description: Based on observation, the center failed to ensure that all electrical outlets shall have protective covers.

Evidence:
1. Outside the office door next to the lighted tree there was one electrical outlet without a protective cover.
2. In the art/lunch room between the two mini refrigerators there was one electrical outlet without a protective cover, and two electrical outlets beside the lunch table without the protective cover.
3. In the literacy classroom there were two electrical outlets beside the table with the ipads without protective covers, and one electrical outlet next to the teacher desk without a protective cover.
4. One outlet cover next to the water fountain did not have a protective cover.

Plan of Correction: Administration will review the standards regarding electrical outlets with staff and complete spot checks in all areas.

Standard #: 8VAC20-780-510-F
Description: Based on review of medication, the center failed to ensure that the medication authorization shall be available to staff during the entire time it is effective.

Evidence:
1. Child #1 had a prescription emergency medication, Epi-Pen, at the center. The medication form expired on 12/21/2022.
2. Child #2 had a prescription emergency medication, Fluticasone HFA, at the center. The medication form expired on 01/05/2022.
3. Administration verified the dates on the medication authorization forms.

Plan of Correction: Administration contacted the parents and requested new authorization forms be completed. In the future administration will put in place a system of completing checks on medication expiration dates and required paperwork.
