Technical Assistance:
Discussed requirements for the JrK class with the Director and Assistant Director.

Comments:
An unannounced monitoring inspection was conducted on 5/2/23 from 11:35am to 2:30pm with the Director. There were 101 children in care, ranging in age from seven-months to five-years-old, supervised by 14 staff. The children were observed singing, reading books, washing hands, eating lunch, and getting ready for nap. Six child records and four staff records were reviewed. The Center has 20 staff with current certification in CPR and First Aid as well as trained in Daily Health Observation. One medication and the corresponding authorization form were reviewed and the center has seven staff current in Medication Administration Training (MAT). Required postings were observed. The attendance, menu, and emergency drill log were reviewed. The most recent Fire Inspection on file was dated 9/29/22 and the most recent Health Inspection on file was dated 12/5/22. Lunch served was turkey in gravy, mashed potatoes, cooked carrots, pineapple, and milk. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 5/11/2023. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 8VAC20-780-130-A
Description: Based on review of six child records, the center did not obtain documentation that each child had received the immunizations required by the State Board of Health before the child attended the center. Evidence: The immunizations on record for Child A, with a start date of 4/11/22, were dated 5/12/22; and the immunizations on record for Child B, with a start date of 10/31/22, were dated 12/28/22.

Plan of Correction: We will ensure all proper forms and immunizations are on file before a child's start date.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center did not ensure that areas and equipment of the center were maintained in a safe condition. Evidence: In the two-year-old classroom, children were observed climbing up on a shelf at the back of the room that was not secured to the wall, creating a toppling hazard.

Plan of Correction: The equipment has been secured to the wall.

Standard #: 8VAC20-780-350-B-4
Description: Based on observation and interview, the center did not maintain the required ratio of staff to children. Evidence: On 5/2/23 there were 17 children observed with one staff in the ?JrK? class for children five-years-old. Interview revealed that the class has been operating at a 1:18 ratio. Until the Summer before eligibility to attend Kindergarten, the required ratio for five-year-olds is 1:10.

Plan of Correction: We will ensure the school age ratio is properly followed when eligibility begins.
