Comments:
An unannounced monitoring inspection was conducted, under the supervision of the Licensing Administrator, on May 8, 2023, from 1:20 p.m. to 2:30 p.m. There were 18 children present, ranging in ages from 18 months to five years old with seven staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.018-A
Description: Based on interviews, the licensee failed to afford the Superintendent reasonable opportunity to inspect their books and records at all times.
Evidence:
1.The Licensing Inspector and Licensing Administrator arrived at the facility at 1:20 p.m.
Staff in the front office confirmed that the records were not available for inspection and would not be available until Staff #1 arrived. Staff #1 arrived at 2:30 p.m.

Plan of Correction: Office staff has been reminded where to access records. Files are being reorganized to make them more easily accessible.

Standard #: 8VAC20-780-270-A
Description: REPEAT VIOLATION
Based on observation, the center failed to ensure that areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition.
Evidence:
1. On 05/08/2023, the following was observed on the toddler playground: one wooden bench with splitting and splintering wood; and standing water in a tire, in two plastic kitchen sinks, and in the tray of the art easel.

Plan of Correction: The wooden bench has been removed and will be replaced with a new bench. Teachers have been reminded to dump out any water that collects during water play.

Standard #: 8VAC20-780-330-B
Description: REPEAT VIOLATION
Based on observations and measurements, the center failed to ensure that where playground equipment is provided, resilient surfacing complies with minimum safety standards.
Evidence
1. The rubber mulch on the large playground was measured to be only one inch deep in the space between the two climbers and only three inches deep near the slide.

Plan of Correction: A maintenance request for a mulch install has been resubmitted to the school business office. The office has informed center staff that they intend to place an order for mulch on 5/19. Date to be corrected: Will have mulch delivery date by 5/22/23.
