Comments:
An unannounced monitoring inspection was conducted today, May 10, 2023 to determine the centers compliance with licensing standards. The inspector was on site from approximately 4:30 pm until 6:00 pm. The site Director was not in attendance during today?s inspection. A substitute coordinator was on site and assisted with the inspection. Later the regional director arrived. There were 16 children present, ranging in ages from five years to 12 years, with two staff supervising. Upon the inspector?s arrival, children were playing on the playground equipment and playing soccer with a staff member. Later children played a group game in the cafeteria and dismissal time was observed with parents signing children out. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition.
Staff interacted with children in a positive manner. A total of five child records were reviewed. Two staff records were reviewed virtually on May 15, 2023. The inspector interacted with staff and children in care.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-60-A
Description: Based you review of five children?s records, the center did not obtain all required information for five children?s records.

Evidence:

1. The record of child #1, enrolled 01/17/2023, did not contain the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached. The record of child #1 also did not contain the work phone number and employer of each parent who has custody.
2. The record of child #2, enrolled 08/29/2022, did not contain the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached. The record of child #2 also did not contain the work phone number of each parent who has custody.
3. The record of child #3, enrolled 08/29/2022, did not contain the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached. The record of child #3 also did not contain the work phone number of each parent who has custody.
4. The record of child #4, enrolled 08/29/2022, did not contain the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached.

Plan of Correction: Per the regional director: We will collect the missing information and look into a way to ensure all required information is obtained.

Standard #: 8VAC20-780-245-L
Description: Based on review of staff records, the center did not ensure there was always at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children.

Evidence:

The record of staff #1 and staff #2 did not contain documentation of completing health observation training. Staff #1 and staff #2 were the only staff on site during the inspection on May 10, 2023.

Plan of Correction: Per the regional director: We will have staff complete the health observation training.

Standard #: 8VAC20-780-540-C
Description: Based on observation, the center did not maintain all required items in the first aid kit.

Evidence:

The first aid kit did not include adhesive tape and an antiseptic cleaning solution/ pad as required.
Staff confirmed the first aid kit did not contain adhesive tape and antiseptic.

Plan of Correction: Per the regional director: We will refill the first aid kit with all required items.
