Technical Assistance:
Consultation provided on sight and sound supervision and the standards regarding allergies, sensitivities and dietary restrictions. Please refer to standards 40M and 60A8.

Comments:
An unannounced monitoring inspection was conducted today with the Administrative staff from 12:10 pm to 2:00pm. Thirty five children were present with five staff providing supervision. The toddlers were observed either on their cots for naptime or finishing up lunch. The older children were outside eating lunch on the picnic tables. The center reports that their are no medications onsite for any of the children. Six injury/ accident reports were reviewed as well as six children and two staff files. The areas of non compliance are outlined on the violation notice.

Violations:
Standard #: 8VAC20-780-40-M
Description: Based on a review of the allergy/sensitivity/dietary restriction list and interview with staff, it was determined that the center is not maintaining a current list of those children who have allergies, sensitivities or dietary restrictions, nor was the list dated.
Evidence:
1. The allergy/sensitivity/dietary restriction list was not dated.
2. Documentation for child A shows that the child has an egg allergy however the allergy list has it as a sensitivity.
3. Documentation for child B shows that the child has an allergy to milk, eggs, peanuts, avocado and melon however the allergy lists has these items as sensitivities.
4. Staff reported that child D has a dairy sensitivity however that information was not on the allergy/sensitivity/diet restriction list.

Plan of Correction: We will revamp our allergy, sensitivity and dietary restriction list to make sure it is accurate and dated.

Standard #: 8VAC20-780-60-A-8
Description: Based on a review of documentation and interview with Administrative staff, it was determined that there were no written allergy care plans for children with diagnosed food allergies.
Evidence:
1. There were no allergy care plans on file for child A, who has an egg allergy or for child B who has a milk, egg, peanut, avocado and melon allergy nor for child C who has a cashew allergy.

Plan of Correction: We will make sure to obtain allergy care plans from a physician for those children who have diagnosed food allergies.

Standard #: 8VAC20-780-70
Description: Based on a review of staff files and an interview with Administrative staff, it was determined that all staff did not have two references on file at the time of hire.
Evidence:
Staff A was hired on 2/16/23 and has no references on file.

Plan of Correction: We will obtain two references.

Standard #: 8VAC20-780-280-B
Description: Based on observation, it was determined that hazardous substances were unlocked.
Evidence:
In the bathroom used by classroom 3, which serves children between 3 to 6 years of age, was a container of sanitizing wipes accessible and unlocked on a shelf.

Plan of Correction: We will remind all staff to keep hazardous substances locked.

Standard #: 8VAC20-780-550-D
Description: Based on a review of the evacuation drill log and interview with Administrative staff, it was determined that the center is not implementing monthly practice evacuation drills.
Evidence:
There was no practiced evacuation drill for January 2023.

Plan of Correction: We will remember to do monthly escape drills.
