Comments:
An unannounced monitoring inspection was completed on May 15, 2023. There were 43 children present with 8 staff supervising. The physical plant, children's records, staff records, first aid kits, evacuation logs, attendance logs and emergency evacuation procedures were reviewed. The children were observed playing floor game, completing table work, preparing to go outside and hand washing. Areas of non-compliance were cited on the violation notice. Standard #: 8VAC20-780-280-B
Description: Based on observation, the center failed to ensure that Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
In the Lion classroom, there were two bottles of Lysol cleaner under the sink.

Plan of Correction: Hazardous substances (Lysol cleaner) was moved to a locked cabinet during the inspection.

Standard #: 8VAC20-780-290-A-3
Description: Based on observation, the center failed to ensure that electrical outlets shall have protective covers that are of a size that cannot be swallowed by children.

Evidence:
In Fish classroom, one outlet cover was missing.

Plan of Correction: Outlet cover was replaced during the inspection.

Standard #: 8VAC20-780-560-G
Description: Based on observation, the center failed to ensure that when food is brought from home, the food container shall be sealed and clearly dated and labeled in a way that identifies the owner.

Evidence:
In the Little Bears' classroom, there were 4 lunch containers that had names on them, but they were not dated.

Plan of Correction: The lunch containers were labeled with the dates during the inspection.
