Comments:
An unannounced monitoring inspection was conducted on 05/15/2023 from 11:10am to 2:35pm. There were 91 children ages 4 months to age 6 years old supervised by 16 staff. The physical plant, outdoor playground area, programming, attendance, menus, 5 staff records, 5 children?s records, 5 medications with corresponding authorization records, emergency drills, injury reports, and policies were reviewed. Children were observed participating in group play, tummy time, nap, lunch and transition activities. Handwashing procedures were also observed. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. There were no violations identified as the result of the self-reported incidents. As a part of the regular monitoring inspection, areas of non-compliance are identified in the Violation Notice.

Description: Based on observation and staff interview, the center did not ensure they post the findings of the most recent licensing inspection of the facility.

Standard #: 8VAC20-780-330-B
Description: 8VAC20-780-330B Resilient Surfacing
Based on observation and inspection of the playground, the licensee did not to ensure that the required resilient surfacing surrounding playground equipment with moving parts or climbing apparatus created a sufficient fall zone.

The Toddler playground has a swing set that is 114 inches in height. At 147 inches from the back of the swing beam, the poured in place resilient surfacing stops and there is a fence installed surrounding the playground. Resilient surfacing is required for the fall zone to extend to twice the height of the swingset on the front and back and resilient surfacing would need to extend to 228 inches from the swing beam to allow a sufficient fall zone.

Plan of Correction: Ensure that swings are removed until back toddler gate is extended at least 228 inches from the swing beam.

Description: Based on interviews and review of 5 staff files, the center failed to ensure that each grouping of children had at least one staff member that met the qualifications of program leader or program director regularly present.

Evidence: The file did not contain documentation and the Director stated that there was no program lead in the Infant A classroom.

Plan of Correction: Ensure that at least one staff member meets the qualifications of Lead teacher is regularly present.
