Comments:

An unannounced monitoring inspection was conducted on May 17, 2023 from approximately 10:50 am to 12:30 pm. There were 26 children in attendance and a total of 6 staff present. 4 staff records and 4 children?s records were viewed. All areas of the center were observed including classrooms, bathrooms, and the playground. The children were observed learning about the weather, listening to music, and eating lunch while the licensing inspector was on-site. The most recent health inspection was conducted on 08/01/22. The most recent fire inspection was conducted on 12/9/22. Evacuation drills were conducted monthly with the most recent on 05/9//23. Violations were cited as a result of this inspection. Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 05/23/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Description:	Based on records review, the center failed to ensure that each child has a physical examination within 30 days after the first day of attendance. Evidence: The record for child #1, date of enrollment, 11/18/22, did not contain documentation of a physical.
Plan of Correction:	Per the center "I will make sure I have a physical before child start."
Standard #:	8VAC20-780-160-A
Description:	Based on a review of staff records, the center failed to ensure that each staff member shall submit documentation of a negative tuberculosis screening 30 days prior to employment and no later than the time of employment and prior to coming into contact with children. Evidence: 1)The record for staff #1, date of employment 01/10/2023, contained the results of a tuberculosis screening dated 05/16/23. 3) The record for staff #4, date of employment, 02/07/2023, contained the results of a tuberculosis screening dated 10/07/2022.

Evidence: The record for staff #1, date of employment 01/10/2023, did not contain documentation of the VDOE course being completed.

Description:	Based on a review of staff records, the center failed to ensure that staff complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.
