An unannounced inspection was conducted on May 2, 2023 from approximately 1:00 pm to 2:00 pm in response to a complaint that was received on April 29, 2023 in reference to ratios. Documentation was gathered and reviewed and interviews were conducted.
The preponderance of evidence gathered during the investigation supports violation of standards in regards to the complaint.
Violations were issued as a result of this investigation.

Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Standard #: 8VAC20-780-350-B-3 Complaint related: Yes Description: Based on documentation review, and interviews, the center failed to follow the staff-to-children ratio specified for 2-year-olds. For two year old children the required ratio isf 1 staff to 8 children. Evidence:
1) Review of Child Supervision (CSR Name to Face) tracking sheets revealed that on the morning of 04/28/2023 classroom #1, with children aged two years, was out of ratio for approximately 48 minutes. There was 1 staff alone from approximately 7:21 am until approximately 8:32 am. At 7:44 am child #9 arrived. Between 7:44 and 8:30am 8 more children arrived bringing the ratio to 17 children, aged 2 to 3 years, with one staff in classroom #1.
2) Review of CSR for classroom #2, with children aged two years, was out of ratio for approximately 9 minutes. There was 1 staff alone from approximately 8:51 am to 9:00 am with 9 children aged 2 years.
3) Interviews with parent #1 revealed that she witnessed 1 staff alone in classroom #1 with approximately 15 children at 8:20 am.
4) The CSR revealed that staff #2 arrived in classroom #1 at approximately 8:32 am. At that time there were 17 children.
5) The Director confirmed that classroom #1 was out of ratio for approximately 48 minutes and classroom #2 was out of ratio for approximately 9 minutes.Plan of Correction: Per the Director "Staff retrained on ratios, policy, and on what to do when out of ratio. Staff retrained on call out procedures. Ensure that scheduling supports student to child ratio."
