Comments:
An unannounced inspection was initiated on 5/17/2023 in response to complaints that were received by the licensing department on 5/9/2023 and 5/19/2023 regarding allegations in the areas of supervision, building maintenance, sanitation, medication, and choking hazards. The inspection was completed on 5/30/2023. On 05/17/2023, the inspector was on site from approximately 8:30 am to 9:50 am. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. Additional interviews were conducted remotely.

The preponderance of the evidence gathered during the investigation supports part of the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Description: Based on a review of documentation and interview, the center did not ensure that non prescription medication was administered by a staff member or independent contractor who meets the requirements in 8VAC20-780-245 J 1 or J 2.

Evidence:
1. During interview, staff stated that Bacitracin, an antibiotic ointment medication, was administered to child #1 by a staff member who did not have the required Medication Administration Training.
2. Administration acknowledged that the medication was administered by a staff who was not MAT certified.

Plan of Correction: admin was not notified of staff member administering until after the fact. Staff are reminded that they cannot take medicine or administer to a child if they are not MAT certified.
