Comments:
A monitoring inspection was initiated and concluded on 5/26/2023. There were 40 children present with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 staff records were reviewed. The inspection started at 11:45 am and concluded at 2:40 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-60-A-8
Description: Based on a review of records and interview, the licensee did not obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence:
Five children's records were reviewed.
The record for child #3 contained documentation that child #3 had a diagnosed food allergy to shellfish.
Child #3's record did not contain an allergy care plan with instructions from a physician regarding the steps to be taken in the event of a suspected or confirmed allergic reaction.
The program director confirmed that this documentation had not been obtained for child #3.
Child #3 has been attending the program since 9/6/2022.

Plan of Correction: The program director contacted child #3's parent following the inspection and requested that an allergy care plan be obtained from child #3's physician and a copy provided to the program. The parent has made arrangements to bring the allergy plan in to the program by the end of the week.

Standard #: 8VAC20-780-260-B
Description: Based on a review of documentation and interview, the licensee was unable to demonstrate that the program obtained annual approval from the health department.
Evidence:
The most recent health inspection available at the time of inspection was dated 5/6/2022.
The program director confirmed that a health inspection had not been conducted within the last 12 month period.

Plan of Correction: The local health department will be contacted next week and a request for a Health Inspection will be made. The request will be documented along with the response and maintained until the health inspection results are obtained.
The program will contact their health department, annually ,as the anniversary date of their inspection approaches to ensure the inspection is conducted in a timely manner.
