Comments:
An announced initial inspection was conducted beginning at 10:00 a.m. and ended at 3:00 p.m. There were 17 children in care under the supervision of 3 staff. A variety of age and stage appropriate supplies were observed for the requested age range of 2 years through 12 years old. Childcare areas were observed. Space was calculated for the classrooms. There is adequate space to accommodate 63 children. Outdoor space and the playground were inspected. Documentation was reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-270-A
Description: Based on observation, not all areas of the center were maintained in clean, safe, operable condition. Evidence: 1. In the three's classroom, there was a wax melt plugged in with hot melted wax inside on the counter accessible to children in care. In the corner of the classroom, above the mini fridge, there were visible cobwebs and dust. The blue rug has rolled up/turned up corners that create a tripping hazard. 2. In the PreK classroom, there is a 16 cube cubby/bookshelf system that is unstable and presents a crush hazard. 3. In the van used for transportation, there were visible food debris, crumbs and dirt all over the floor in between the two back seats. There were two expired booster seats. 4. On the two's playground, there were16 screws on the fencing that extended from 6 threads to 18 threads long that created scratching and impalement hazards. There were leaves and debris observed underneath the Step2 slide/climber that created a potential for animal/reptiles. There was an unstable umbrella on a stand that created a crush hazard. 5. On the PreK/School Age playground, the helicopter climber had two loose bolts that created an entanglement hazard. There were 8 screws on the fencing that extended from 9 to 18 threads long that created a scratching or entanglement hazard. There was a mesh bag that was tangled and tied to the fencing that created and entanglement hazard. On the stairs of the large climber, the stripping material is worn and broken in areas creating a tripping and scratch hazard.

Plan of Correction: The wax melt was immediately unplugged and removed from the classroom.

Description: Based on observation of the center's playground, including measurements in several areas, it was determined that the resilient surfacing did not meet requirements of at least six inches in all use and fall zones of the playground equipment. Evidence: At the two feet six inches slide chute, resilient surfacing was measured at three inches. In several use/fall zones, resilient surfacing was measured at four inches. Grass and weeds were observed growing in several areas of the mulch. A minimum of six inches of wood chips/mulch is required for the playground equipment.
