An unannounced complaint inspection was initiated on May 8, 2023 to investigate a complaint that was received by the Division on April 28, 2023.
Based on staff interviews and review of staff files, the allegation made by the complainant regarding staff training was determined to be valid.

Standard #: 8VAC20-780-240-A Complaint related: Yes Description: Based on review of five staff files and staff interview, the facility failed to have 2 out of 5 staff complete the Virginia Department of Education-sponsored orientation course (VA Pre-service) within 90 days of employment. Evidence: Staff 3 (date of employment 11/30/2021) and Staff 4 (date of employment 6/09/2022) did not have documentation of having completed the VA Dept of Education sponsored orientation course within 90 days of employment. Management stated that the staff did not have documentation of the training. Plan of Correction: All staff will complete the 10 hour pre-service training within 30 days of employment.
Standard #: 8VAC20-780-530-A-1 Complaint related: Yes Description: Based on staff interview and record review, the facility failed to have one staff in each grouping of children with cardiopulmonary resuscitation (CPR). Evidence: Staff 1, Staff 2, Staff 3, Staff 4, and Staff 5 were on site with 27 children on 5/08/2023. None of the 5 staff have documentation of current CPR on file. Management stated that 5 out of 5 staff do not have current CPR certification. Plan of Correction: All staff will get first aid and CPR certification.
Standard #: 8VAC20-780-530-A-2 Complaint related: Yes Description: Based on staff interview and record review, the facility failed to have one staff in each grouping of children with first aid certification. Evidence: Staff 1, Staff 2, Staff 3, Staff 4, and Staff 5 were on site with 27 children on 5/08/2023. Only Staff 3 has documentation of current first aid certification on file. Management stated that 4 of the 5 staff are missing first aid certification. Plan of Correction: All staff will get first aid and CPR certification.
