22.1 EARLY CHILDHOOD CARE AND EDUCATION

Comments:
A renewal inspection was conducted today, June 5, 2023. The inspector was on site from approximately 11:00 am until 1:00 pm. The director was present and assisted in the inspection. There were 29 children present, ranging in ages from three years to twelve years, with six staff Supervising. Children were observed in a variety of activities including outdoor play with balls, decorating letters with beach stickers, and painting plants to go along with the inter plant theme. Later children had their lunch outside on the picnic tables. Children provide their own meals which were labeled with the child?s name and today?s date. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and seven staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-A
Description: Based on review of seven staff records and interview, the center did not ensure one staff obtained background checks every five years.

Evidence:

1. The record for staff #2 (employed 04/30/2018) contained the results of a fingerprint national criminal record check dated 04/27/2018, exceeding five years.
2. The director confirmed a request to update the fingerprint national criminal record check had not yet been obtained.

Plan of Correction: Per the director: We will schedule the fingerprint appointment for staff to ensure they are scheduled and completed in a timely manner.

Standard #: 8VAC20-780-70
Description: Based on review of seven staff records, the center did not obtain documentation that two or more references as to the character and reputation were checked before employment for one staff member.

Evidence:

1. The record of staff #1, employed 05/09/2023, contained documentation of only one reference when at least two references are required.
2. The director acknowledged only one reference was documented in the record.

Plan of Correction: Per the director: We will obtain a second reference for the staff member.

Standard #: 8VAC20-780-550-P
Description: Based on review of 13 written children?s incident/ accident reports, the center did not ensure all required information was documented for four of the incident/ accident reports.
Evidence:

Four incident reports dated 03/21/2023, 04/05/2023, 04/11/2023 and 05/12/2023 did not contain the following required information; date and time parent was notified of the injury, and documentation on how the parent was notified.

Plan of Correction: Per the director: We will remind teachers to fill out accident reports completely and administration will double check to ensure they are complete.
