An announced initial inspection was conducted on 06/05/2023 from 1:35 p.m. to 2:15 p.m. The inspector reviewed compliance in the areas physical plant, programming, special care and emergencies and nutrition.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-270-A Description: Based on observation and interview, the center failed to ensure that areas and equipment of the center were maintained in a clean, safe and operable manner.
Evidence:
1. In the main classroom, there was one ceiling tile that was sagging, one ceiling tile that was missing, one vent missing a cover and one floor tile that was partially missing.
2. Staff #1 confirmed the ceiling tile sagging, another ceiling tile and the vent cover were missing, and the floor tile was partially missing.Plan of Correction: Not available online. Contact Inspector for more information.
