Comments:
An unannounced monitoring inspection was initiated and concluded on 6/9/2023. The inspector was on site from approximately 9:20 am-11:30 am. There were 39 children present, ranging in ages from 2 months to 11 years, with 8 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies, nutrition and background checks. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on a review of five (5) staff records and interview, the center did not ensure to obtain documentation of a negative tuberculosis screening within the required time frame for one (1) staff. 1. Documentation of the screening must be submitted at the time of employment and prior to coming into contact with children. 2. The documentation must have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Evidence:
1. The record of staff #3 (date of employment: 4/17/2023) contained a negative TB screening dated 5/30/2022.
2. Administration acknowledged that the TB screening was submitted prior to employment but was not completed within the last 30 calendar days of the date of employment.

Plan of Correction: This staff has an appt for another TB screening on 6/13/2023.

Standard #: 8VAC20-780-70
Description: Based on a review of five (5) staff records and interview, the center did not ensure that a complete record was kept for one (1) staff.

Evidence:
1. The record of staff #4 (date of employment: 6/5/2023) did not contain documentation that two or more references as to character and reputation as well as competency were checked before employment. If a reference check is taken over the phone, documentation must include:a. Dates of contact; b. Names of persons contacted; c. The firms contacted; d. Results; and e. Signature of person making call.
2. The record contained documentation that calls were attempted prior to employment but the results were not documented.
3. Administration acknowledged that the results of the phone reference checks were not obtained.

Plan of Correction: Follow up calls were made on 6/12/23 and references were obtained.
