Comments:
A renewal inspection was initiated on 05/24/2023 and concluded on 05/24/2023. There were 62 children present, ranging in ages from 6 months to 5 years, with 15 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 15 staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Please complete the plan of correction and date to be corrected sections for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s): 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Violations:
Standard #: 8VAC20-780-280-B
Description: Based on classroom observation, the center failed to ensure that hazardous substances were kept in a locked safe place using a safe locking method that prevents access by children.

Evidence:
1. Bottle of bleach water labeled was left out on the sink in the bathroom of the two year old classroom.
2. The sink is accessible to the children and where they can wash their hands.

Plan of Correction: To have all teachers ensure that all chemicals are locked away immediately after use as well as properly labeled.

Standard #: 8VAC20-780-520-A
Description: Based on a review of the over-the-counter skin products, the center failed to ensure that over-the-counter skin products shall not be kept over the expiration date.

Evidence:
1. There were two tubes of expired Bordeaux's Butt Paste. First tube expired 01/2023 and the second tube expired 03/2023.
2. The staff confirmed the expiration date of the Bordeaux's Butt Paste.

Plan of Correction: Have teachers to check all over-the-counter skin products.
