Comments:

An unannounced monitoring inspection was conducted on 6/1/2023. There were 41 children, ages 3 years-12 years, 12 staff and 2 administrative staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. Medications for children were observed during the inspection. A total of 4 children?s records, 3 staff records and 8 medications were reviewed. The children were observed during lunch time and nap time. The inspector arrived for the inspection at 11:45am and departed the center at 1:23pm. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #:	8VAC20-780-150-B
Description:	Based on review of four children?s records, the center failed to ensure that all immunizations forms obtained were signed by a physician. Evidence: Child #4 first date of attendance was 5/30/2023 and on the day of the inspection the immunization form that was in the child?s record was not signed by a physician.
Plan of Correction:	Provider will obtain a signed immunization form from the parent.
Standard #:	8VAC20-780-90--A
Description:	Based on the review of four children?s records, the center failed to ensure that a written agreement between the parent and the center shall be in each child?s record by the first day of attendance. Evidence: Child 4 first date of attendance was 5/30/2023 and on the day of the inspection the registration form did not have a statement that the center will notify the parent when a child becomes ill.
Plan of Correction:	Provider will add a statement concerning the center notifying parents if a child becomes ill to the summer camp registration form.
