Comments:
An unannounced monitoring inspection was conducted in the center on June 5, 2023 from 12:35 PM until 3:25PM and concluded on June 6, 2023. At the time of entrance, 72 children were in care with 13 staff members present. The sample size consisted of 7 children?s records and 7 staff records. Children were observed eating lunch, during rest period and outdoor play time. Areas of non-compliance are identified on the violation notice?.

Violations:
Standard #: 8VAC20-780-260-B
Description: Based on documentation review and staff interviews, the licensee did not ensure to obtain an annual approval from the health department.
Evidence: Center Director confirmed the most recent copy of the health inspection available for review was dated 5-16-2022.

Plan of Correction: Contact health department to schedule new inspection

Standard #: 8VAC20-780-330-A
Description: Based on measurements, observation and staff interviews, the licensee did not ensure where playground equipment is provided, there shall be under equipment with moving parts or climbing apparatus enough resilient surfacing to create a fall zone free of hazardous obstacles. Fall zones are defined as the area underneath and surrounding equipment that requires a resilient surface. A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use. Falls zones shall not include barriers for resilient surfacing.
Evidence: The following inadequate fall zones were observed on the playgrounds.
1. On the playground the climbing wall closest to the building does not have the required fall zone as the following items are within the 6 feet of the climbing wall.
a) There is a plastic caterpillar is 3?10? from the climbing wall
b) The plastic playground boarder is 3? 6? form the climbing wall
c) The metal fence is 4? 6? from the climbing wall.
2. On the playground the climbing wall closest to the woods does not have the required 6 feet of fall zone because there is a plastic caterpillar that is 3?4? from the corner of the climbing wall.

Plan of Correction: The caterpillar will be moved to the grass and the climbing wall will be closed for now.

Standard #: 8VAC20-780-340-D
Description: Based on staff record review, observation and staff interviews, the licensee did not ensure there shall be in each grouping of children at least one staff member who meets the qualifications of a program leader or program director.
Evidence: Staff #8 and Staff #9 were observed working in the toddler room with 8 toddlers. Center Director confirmed through video footage and interviews that the Toddler room did not have a qualified program lead teacher on the day of the inspection from approximately 8:30 AM until 11:30 AM as required. Center Director verified there was not documentation indicating program lead qualifications for Staff #8 or Staff #9.

Plan of Correction: Staff member #9 will be lead qualified

Standard #: 8VAC20-780-560-F
Description: Based on observation and staff interviews, when centers choose to provide meals or snacks, they shall follow the most recent, age-appropriate nutritional requirements of the Child and Adult Care Food Program of the United States Department of Agriculture (USDA).
Evidence: Center director confirmed the most recent, age-appropriate nutritional requirements of the (USDA) were not followed for serving lunch in Pre-K rooms #B106 and #C106, age range of children 3-5. Pancakes, sausage, yogurt and milk were the only items observed served for lunch. The USDA requires at a minimum ? cup of fruit and ? of vegetables to be included in the items served for lunch.

Plan of Correction: Retrain kitchen staff on serving alternative menu items when out of menu items
