An unannounced monitoring inspection was conducted on June 8, 2023 from approximately 11:00 am to 12:30 pm. There were 74 children in attendance and a total of 13 staff present. 7 staff records and 7 children?s records were viewed. All areas of the center were observed including classrooms, bathrooms, and the playgrounds. The children were observed eating lunch, listening to a story about dinosaurs, and playing with table toys while the licensing inspector was on-site.
Evacuations were conducted monthly with the most recent on 06/02/23. The most recent health inspection was done on 06/06/23. The most recent fire inspection was done on 02/15/23.
Violations were cited as a result of this inspection.
Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 06/15/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Standard #: 8VAC20-780-160-A Description: Based on records review, the center failed to ensure that each staff member shall submit documentation of a negative tuberculosis screening within the last 30 calendar days of the date of employment.
