Comments:
An unannounced focused inspection was conducted on 6/14/23 to assess compliance with specific standards identified in the Intensive Plan of Correction submitted. Two inspectors were onsite from approximately 12:30 pm to 3:00 pm on 6/14/23. There were a total of 17 children in care ranging from 7 months to 5 years of age with four staff supervising. The center director was not present during the on-site inspection.

During the inspection, the children were observed finishing lunch, handwashing, toileting and transitioning to nap. Staff?s interactions with the children were appropriate. The center was furnished with supplies and age appropriate toys for children in care. The inspector reviewed compliance in the areas of administration, physical plant, and special services.

The records for five children were reviewed and an inspection of the center was conducted. Information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Just writing the word corrected is not
acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

Standard #: 8VAC20-780-140-A
Description: Based on review of records, the center failed to ensure each child completed a physical examination by or under the direction of a physician before the child?s attendance or within 30 days after the first day of attendance.

Evidence:
At the time of review, the record for Child #2 (date of attendance: 04/25/2023) did not
document a physical.

Standard #: 8VAC20-780-60-A
Description: Based on review of records, the center failed to ensure records for each child contained the
required information.

Evidence:
1. The record for Child #1 (date of attendance: 4/18/23) failed to include a
second emergency contact person to contact if a parent cannot be reached.
2. The record for Child #2 (date of attendance: 4/25/2023) failed to include; written
agreements between the parent and the center as required by 8VAC20-780-90, a
second emergency contact person to contact if a parent cannot be reached, and the address for the one emergency contact person documented.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to ensure areas and equipment of the center,
inside and outside, were maintained in a clean, safe and operable condition.

Evidence:
1.The gate leading to the preschool room had a broken piece, creating jagged edges that could snag the skin of a child.
2. Three mats that children were observed using during nap time were torn with exposed foam.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center failed to ensure hazardous substances such as cleaning material, insecticides, and pesticides were kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1.The open storage area in the preschool room where children were observed, contained two bottles of arm and hammer laundry detergent with one bottle being within reach of children. 2.The bottles contained the warning, CAUTION: keep out of the reach of children.
