Technical Assistance:
Review of the center?s safe sleep policy determined it is not consistent with safe sleep practices. ? A child Care Provider's Guide to Safe Sleep, Helping to reduce the risk of SIDS?, which states ?Loose bedding, such as sheets and blankets, should not be used. Sleep clothing, such as sleepers, sleep sacks, and wearable blankets, are good alternatives to blankets.? was provided to the center with the recommendation that the safe sleep policy be revised to be consistent with safe sleep practices.

Comments:
A renewal inspection was initiated on 06/21/2023 and concluded on 06/21/2023 from 10:25 AM to 1:45 PM. There were 140 children present, ranging in ages from 3 months to 12 years, with 20 staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies. A total of seven children?s records and the five staff?s records and three board officer?s background checks were reviewed for updates.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22.1-289-036-B-4
Description: Based on record review and interview, the center failed to obtain an out of state child abuse and neglect search within 30 days.

Evidence:

1. Board officer 1 completed an updated sworn disclosure on 6/30/21 which disclosed the individual moved to a new state.
2. Documentation in the file showed the child abuse and neglect search for that state was mailed and returned on 12/4/20 with errors stating the search could not be completed.
3. The director stated the corrections were not made and the background check was not remailed.

Plan of Correction: The director worked on completing the background check during the inspection and will follow up if results are not received within 30 days. The results will be forwarded to the inspector upon receipt. A system will be put into place to remind the director to complete background checks and follow up to ensure the results are received.

Standard #: 8VAC20-780-40-M
Description: Based on observation and interview, the center failed to ensure the written list of children's allergies, sensitivities and dietary restrictions were maintained in a way that was accessible to staff who work with children and kept confidential in each room or area where children are present.

Evidence:

1. The teacher in the 16 month old classroom stated the list of children's food allergies and sensitives were listed on the white board.
2. On the white board in the classroom were the names of two children and the foods they are sensitive and allergic to.

Plan of Correction: The allergies and sensitives were taken off the white board during the inspection. The staff members in the classroom were reminded that this information is confidential and should only be kept in locations for staff to see such as the clipboard.
