Comments:

A monitoring inspection was conducted on June 23, 2023 with the center director and center staff. There were 59 children present, ranging in ages from6 months to 5 years, with 13 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 2 staff records were reviewed. The children were having rest time and afternoon snack. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #:	8VAC20-780-130-A
Description:	Based on review of six children?s records, the facility failed to have the immunization record for each child before the child can attend the center. Evidence: Child 3 (date of enrollment 1/25/2021) did not have documentation of an immunization record until 9/16/2021.
Plan of Correction:	Immunization records will be requested prior to the child's first day.
Standard #:	8VAC20-780-140-A
Description:	Based on review of six children?s records, the facility failed to have a physical examination for each child within 30 days of attendance. Evidence: Child 3 (date of enrollment 1/25/2021) did not have documentation of an physical on file until 9/16/2021.
Plan of Correction:	The physical examination will be requested before the child's 30th day of enrollment.
Standard #:	8VAC20-780-450-A
Description:	Based on observation, the center did not ensure that cots or mats used by children during the designated rest period shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges. Evidence: In the Island classroom, two children did not have a top or bottom cover during the designated rest time.
Plan of Correction:	All children will be supplied with a top and bottom cover at rest time.
Standard #:	8VAC20-780-500-B
Description:	Based on observation, the center did not ensure that the disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. In the Ocean room bathroom, there were diapers in a trash can that did not have a lid.
Plan of Correction:	Diapers will be disposed of in the proper disposal system.

Description:	Based on review of the emergency drill log, the facility failed to conduct evacuation drills monthly. Evidence: the facility did not have documentation of an evacuation drill in 4/2023 or 5/2023.
Plan of Correction:	Evacuation drills will be conducted and documented monthly.
