An unannounced, non-mandated monitoring inspection was conducted on 06/21/2023 in response to the facility's self-reported incident of the possible use of forbidden action and/or inappropriate discipline techniques by staff. The investigation included observation and staff interview. A preponderance of evidence did not support the allegations of non-compliance with applicable Standards or law and no violations were cited.
Results of the investigation were reviewed with the program director during the exit interview.
