1. Reminded staff that electrical outlets in areas accessed by children are to have protective covers.
- Comments:

Conducted an unannounced renewal inspection at 12:00pm. Observed 38 children + 8 direct-care staff. Ratios and supervision were in compliance. Children were engaged in a variety of activities to include: singing, educational activities, show + tell, eating snack, taking naps, and departing for the day. Staff were actively engaged with the children and activities. The facility was found to be well maintained and supplied with toys and equipment for the children. Staff demonstrated prompt responses in managing situations involving children that did not feel well today and gave each child 1:1 attention until the parents arrived. Standard #: 8VAC20-780-140-A Description: Based on records reviewed, it was determined that 1 of 2 children?s records did not include a physical exam within the first 30-days of attendance. Evidence: Child #5-The child began attending in August 2022 and to-date there is no physical exam on-file. Plan of Correction: The parents have been contacted and the information will be completed. The child is not in attendance until the physical exam has been provided to the school.
Standard #: 8VAC20-780-70 Description: Based on records reviewed, it was determined that 1 of 3 staff records did not include required information. Evidence: Staff #5-The record did not include documentation of 2 reference checks. Date of hire was August 2018. Plan of Correction: We think the reference checks were mistakenly archived. New reference checks were obtained on 2/16/23 and 2/17/23.
Standard #: 8VAC20-780-240-A Description: Based on records reviewed, it was determined that 1 of 3 staff records did not include documentation of completion of the VDOE sponsored orientation course within 90-days of employment. Evidence: Staff #7-Date of hire was in August 2022, and to-date there was no documentation of the training being completed. Plan of Correction: Staff #7 completed the training course on 2/20/23.
