A monitoring inspection was initiated on 06/27/2023 and concluded on 06/27/2023. There were 69 children present, ranging in ages from 6 weeks to 11 years, with 13 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 8VAC20-780-160-A Description: Based on record review, the center failed to ensure that a negative tuberculosis test/screening was submitted at the time of employment and prior to coming into contact with children.
Evidence:
1. The record of Staff 1 (DOH 06/15/2023) did not have documentation of a negative tuberculosis test/screening result. Staff 1 was working in the classroom when the Licensing Inspector was completing observations.
2. The record of Staff 2 (DOH 06/15/2023) did not have documentation of a negative tuberculosis test/screening result. Staff 2 was working in the classroom when the Licensing Inspector was completing observations.
3. The record of Staff 3 (DOH 06/15/2023) did not have documentation of a negative tuberculosis test/screening result. Staff 3 was working in the classroom when the Licensing Inspector was completing observations.Plan of Correction: I thought we had time to get it after they were hired. I will get them to get the TB screenings this week.
