Technical Assistance:
Hazardous substances such as bleach water shall be kept in a locked place using a safe locking method that prevents access by children.

Discussed ensuring that bottles provided for infants during drop off are put in the refrigerator as soon as possible.

Comments:
An unannounced monitoring inspection was conducted from 10:15am - 12:30pm with the center director. There were 69 children, ranging from five months to six years of age, present with 14 staff supervising. Infant care was observed and older children were engaged in table toys, center time, circle time, playing on the playground and in the gym. Four child records and four staff records were reviewed, along with the emergency drill log, written daily attendance, written allergy list, menu, and required posted information.

Description: Based on review of four staff records, the facility did not have an original central registry report within 30 days of employment for all staff. Evidence: the record for staff #4 (date of hire: 5/19/2023) did not contain documentation of a central registry search report.

Standard #: 8VAC20-780-160-A
Description: Based on review of four staff records, not all staff had documentation of a TB test screening completed within the last 30 calendar days before employment. Evidence: the record for staff #1 (date of hire: 3/13/23) had a TB test/screening dated: 1/11/2023. The record for staff #3 (date of hire: 5/31/2023) had a TB test/screening dated: 4/13/2023 that was not signed by a physician, physician?s designee or an official of the local health department.

Description: Based on review of documentation, and an interview with staff, the facility failed to maintain in a way that is accessible to all staff who work with children, a current written list of all children?s allergies, sensitivities and dietary restrictions. This list shall be dated and kept confidential in each room or area where children are present. Evidence: There was no allergy list present with the toddler class playing in the gym. Staff stated that the list was on the clipboard left in their classroom.

Description: Based on review of documentation, and an interview with staff, the facility failed to maintain a written record of attendance that documents the arrival and departure of each child in care as it occurs. Evidence: The attendance in the "Get Set" classroom listed 12 children however 13 children were observed in care.

Description: Based on observation, the facility failed to ensure that children wash their hands with soap and water or disposable wipes before and after eating. Evidence: In the infant classroom one infant was observed drinking a bottle and their hands were not washed beforehand and a second infant did not have their hands washed after drinking a bottle (of milk).

Standard #: 8VAC20-780-500-B
Description: Based on observation and staff interview, the facility failed to ensure that all diapering procedures, including using a nonabsorbent surface (changing table) for children under three and disposing of diapers in a leakproof receptacle that is foot-operated, are followed as required. Evidence: 1) there is no changing table in the "Get Set" classroom which includes children 24-30 months of age. Staff stated that they change children in the bathroom and diapers were observed in trashcans in the bathroom. 2) A foot operated trash can lid in the infant class has a locking mechanism that requires unlocking it by hand before the foot pedal will open the lid. 3) A dirty diaper and wipes were observed in a trash can that was not foot-operated in the "First Steps" classroom. 4) Staff in the preschool classroom stated that pullups go into a trash can in the bathroom that is not foot operated.
