Technical Assistance:
The required facility's written consistency of care policy (8VAC20-780-350.F) was discussed with the program director.

Comments:
A monitoring inspection was conducted on 7/6/2023. There were 109 children present, ranging in ages from infants to school age, with 17 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 10 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director during the exit interview.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based upon observation, the facility has not ensured that areas outside of the center are maintained to be safe.
Evidence:
The concrete ground supports for the fence at the eastern end of the preschool playground have become uncovered in three areas due to soil erosion, creating a potential tripping and injury hazard.

Standard #: 8VAC20-780-280-B
Description: Based upon observation, the facility has not ensured that hazardous substances are kept in locked places using safe locking methods that prevent access by children.
Evidence:
1. The cabinet under the changing table in the Seahorse classroom was not locked. There were several diaper creams and a bottle of hand sanitizer with warning labels in this cabinet.
2. A spray bottle labeled bleach water was hanging from the high shelf above the sink in the Seahorse classroom.
3. There was an unlocked closet in the Dolphins classroom in which was a container of disinfecting wipes with a warning label.

Description: Based upon observation, the facility has not ensured that where playground equipment is provided, resilient surfacing complies with minimum safety standards and shall be under climbing apparatus to create a fall zone free of hazardous obstacles.
Evidence:
The wood mulch used as resilient surfacing under the large climbing structure on the preschool playground has decomposed, leaving only a one to two inch layer of mulch under the structure and at the base of the slides.

Description: Based upon observation, the facility has not ensured that materials are of an appropriate size for the child using it.
Evidence:
The children in the older infant classroom were observed playing with small, 1 1/2 inch long, cylindrical building logs presenting a potential choking hazard. Several children were placing the small logs in their mouths.

Standard #: 8VAC20-780-500-B
Description: Based upon observation, the facility has not ensured that diapers are disposed in a storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches the exterior of the storage system.
Evidence:
1. The lid on the step-down diaper pail in the Angelfish classroom does not open wide enough for a diaper to be disposed without touching the lid.
2. The lid on the diaper pail in the older infant classroom does not open when stepping upon the foot pedal.
