An unannounced complaint inspection was initiated in the center on June 12, 2023 and completed on July 11, 2023 regarding concerns of Staffing/ Supervision and Administration. There were
observations of the afterschool program to include children transitioning from different activities. During the inspection the following were completed: interviews with administrative staff, center staff, a review of center documentation, observation of the classroom and area in which the incidents occurred. Additionally, there were staff and collateral interviews, and a review of pertinent documentation during the investigative process.
The preponderance of the evidence gathered during the investigation does support the allegation related to Supervision and Administration and therefore, the complaint is determined to be valid.
The inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-40-E Complaint related: Yes Description: Based on a review of the center's documentation and interviews, the licensee did not ensure the operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.
Evidence: The center staff did not follow their own policy related to playground safety procedures. Staff #1 confirms they did not follow the center's playground policy of ?count children before you leave the playground?. Staff #1 did not count children until after they came inside from the playground.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-340-F Complaint related: Yes Description: Based on interviews and documentation review, the licensee did not ensure all children under 10 years of age shall always be within actual sight and sound supervision of staff.
Evidence: Staff #1 confirmed that on June 2, 2023 at approximately 3:50 PM, two children (Child #1 and Child #2) both age 5, were left on the fenced in playground for approximately 1 to 2 minutes without supervision.
Staff #1 confirmed, that they asked children to line up and come inside from the playground and then the outside playground door closed.
Staff #1 stated ?I started check the sign-in sheet and count children after everyone came inside and then I heard knocking on the door and I opened the backdoor and two children (Child #1 and Child #2) were outside?.Plan of Correction: Not available online. Contact Inspector for more information.
