Comments:
A center administrative self report other inspection was initiated on 7/14/23 and concluded on 7/17/23. The inspector reviewed compliance in the areas of administration, staffing and supervision, medication, and special care and emergencies. The inspection started at 12:45pm and concluded at 9:00am.

The Information reported and verified during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-245-J
Description: Based on an administrative center self report the center failed to ensure emergency medications were administered by an appropriately certified staff member as required.

Evidence:

Center administration self-reported that staff #1 has been administering an emergency medication (insulin) to a child since June 2023 when public schools released for the summer, through 7/14/23.

Staff #1 has the required MAT training, but does not have the add on portion for emergency medication administration as required.

Staff #1 and center administration stated that the parent of the child did give permission for administration by this staff and also came in and showed the staff how to administer the medication, but that the staff member has only the regular MAT certification at this time.

Plan of Correction: The emergency medication trained staff has been working, since public school has been released for the summer. at the school age site.
Administration will have this staff member come to the other site to administer this emergency medication until another staff member can obtain the certification to ensure compliance.

Standard #: 8VAC20-780-350-B-4
Description: Based on a center administrative self report the center failed to ensure ratios were met for the grouping of children 3 years-5 years as required.

Evidence:

The center administration self reported that on the morning of 7/13/23, from approximately 10am-10:30am, the ratio was not met for the 3-5 year group on the playground, while a brief lead teacher meeting occurred.

Center stated there were a total of 36 children, aged 3-5 years, with 2 staff at the time. Center also stated that there were six 2 year olds also on the playground, with 1 staff.

The required ratio for the grouping of 3-5 year olds is 1 staff to 10 children. Therefore, non-compliance occurred for this grouping of children as reported by the center administration.

Plan of Correction: An impromptu lead teacher meeting was held in the back room, that allows site onto the playground, for approximately 30 mins.
Ratios were met immediately after this meeting, and the children and staff on the playground at the time could be seen from the room where this meeting was happening.
Ratios will be followed to ensure compliance.
