Description: REPEAT VIOLATION

Based on record review, the center failed to obtain a copy of a child abuse and neglect search and criminal record check from any state in which the individual has resided in the preceding five years.

Evidence: The record for Staff 9 contained documentation that Staff 9 has resided outside of the state of Virginia within the preceding five years. There was not an out of state child abuse and neglect registry check from all of the states in which the individual has resided in the preceding five years available for review during the inspection for Staff 9. The documented hire date in the Record for Staff 9 was 03/23/2023.

Plan of Correction: Will request the information.

Description: Based on record review, the center failed to ensure that all applicants shall undergo a background check in accordance with the Code of Virginia prior to employment and every five years thereafter.

Evidence:
1. The most recent sworn statement in the record for Applicant 1 was dated 10/04/2017; there was not a follow-up sworn statement obtained by 10/04/2022.
2. The most recent sworn statement in the record for Applicant 2 was dated 08/21/2017; there was not a follow-up sworn statement obtained by 08/21/2022.
3. The most recent search of the central registry in the record for Applicant 2 was dated 11/07/2017; there was not a follow-up search of the central registry obtained by 11/07/2022.
4. The most recent search of the central registry in the record for Applicant 3 was dated 11/07/2017; there was not a follow-up search of the central registry obtained by 11/07/2022.

Standard #: 8VAC20-780-130-E
Description: Based on record review, the center failed to obtain documentation of additional immunizations once every six months for children under the age of two years.

Evidence: The record for Child 5 contained a documented start date of 12/15/2020. The most recent immunization record in the record for Child 5 was dated 11/18/2021; additional immunizations were not obtained every 6 months from 11/18/2021 until the child turned 2 years old.

Plan of Correction: Will get the updated records.

Standard #: 8VAC20-780-160-A
Description: Based on record review, the center failed to ensure that each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children.

Evidence: The record for Staff 5 contained a documented hire date of 04/05/2023. The tuberculosis screening in the record for Staff 5 was dated 06/21/2023. The tuberculosis screening had not been completed prior to coming into contact with children.

Plan of Correction: Will get the results prior to contact with children in the future.

Standard #: 8VAC20-780-60-A
Description: Based on record review, the center failed to ensure that the separate record for each enrolled child shall contain all of the elements as required by the standards.

Evidence:
1. The record for Child 2 did not contain the following information: a written care plan for a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction (the record contained documentation of a confirmed food allergy).
2. The record for Child 5 did not contain the following information: the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached (the addresses for both designated people were incomplete).

Plan of Correction: Will contact the parents and get the information.

Standard #: 8VAC20-780-560-F-4
Description: Based on observations, the center failed to ensure that when centers choose to provide meals or snacks, children three years of age or younger may not be offered foods that are considered to be potential choking hazards.

Evidence: Children ages 16 months-3 years were observed eating whole corndog nuggets for lunch. The hotdog inside of the corndog breading is considered to be a potential choking hazard.

Plan of Correction: Will cut the corndogs nuggets up or will no longer serve them.
