Technical Assistance: When children at the center have been exposed to a communicable disease listed in the Department of Health's current communicable disease chart, parents shall be notified within 24 hours or the next business day of the center's having been informed unless forbidden by law, except for life threatening diseases, which must be reported to parents immediately. Discussed with the Director that this may need to be posted as well as emailed to parents so parents do not miss the information. The center needs to confirm that all parents are informed, even if their child is out during the time the information is posted at the center.

Comments:
An unannounced complaint investigation was conducted on 7/19/23 from 11:05am to 12:35pm in response to a complaint received in the licensing office regarding allegations related to preventing the spread of disease and cleanliness in the center. Based on observation and interview, the allegations were determined to be valid and violations were cited. Other violations not reported in the complaint were also found during the investigation and are included in the Violation Notice. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 7/27/23. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Complaint related: Yes
Description: Based on observation, the center did not ensure that areas and equipment of the center were maintained in a clean condition. Evidence: In the ?Ducks? class for three- to four-year-old children, multiple large stains were observed on the carpet.

Plan of Correction: Professional cleaners are hired to do deep cleaning of carpets and floors.

Complaint related: No
Description: Based on observation, the center did not ensure that hazardous substances, such as cleaning materials, were kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. On 7/19/23 at approximately 11:23am in the ?Ducks? class for three- and four-year-old children, two spray bottles labeled ?mild bleach? and two spray bottles labeled ?strong bleach? were observed in an unlocked high cabinet over the sink. The staff member confirmed that there was no lock for the cabinet. 2. At approximately 11:34am in the ?Frogs? class for two-year-old children, a spray bottle labeled ?strong bleach? was observed on a high, open shelf in the bathroom. An aerosol disinfectant spray was also observed on the changing table in the bathroom.

Plan of Correction: 1. New locks are placed on all cabinets to keep the class safe. 2. Staff member is redirected to do training and keep the hazardous material in the locked cabinet.

Description: Based on observation, when staff were supervising children, they did not always ensure their care and protection. Evidence: 1. On 7/19/23, between approximately 11:05am and 12:30pm, washcloths were observed hanging on the faucets in the sinks or lying next to the sinks in the classrooms for infants, toddlers, three- and four-year-olds, four- and five-year-olds, and schoolagers. Staff in the infants and toddler rooms were observed wiping down counters with the washcloths and placing them back on the faucets to be used again. A staff member in the infants was observed drying their hands on the washcloth and placing it back next to the sink. While paper towels were available at the sinks in each classroom, there were no used paper towels in the trash cans in the infant classroom or two-year-old classroom or bathroom. The staff member in the twos reported that the trash had not been taken out for the day yet. Disposable towels are to be used to dry hands after washing, as well as to wipe down areas of the classroom, as washcloths can hold and spread germs. 2. In the ?Ducks? class for three- to four-year-olds, at approximately 11:22am, a staff member was observed passing out milk and opening bananas for children with gloves on. The staff member then opened the step-on trash can with their gloved hand to throw away a banana peel, and then continued passing out milk with the same gloves after touching the trash can lid. A child then came over and opened the trash can with their hand to throw away their plate then went back to the table to continue eating their food from home without washing hands.

Plan of Correction: 1. All the staff members are doing the VA Annual Health and Safety training 2023 Update again. 2. Wash cloths are removed. Staff members are ensuring the use of disposable paper towels for cleaning, wiping and drying purposes. 3. Staff members and kids are encouraged and redirected on frequent hand washing and to observe recommended cleaning and hygiene practices.

Complaint related: Yes
Description: Based on observation, staff did not ensure that a diapering surface was cleaned with soap and at least room temperature water and sanitized after each use. Evidence: At approximately 11:45am, a staff member in the Infant class was observed changing a child?s diaper. The staff then moved the child to a highchair to feed them and did not clean or sanitize the diaper changing pad.

Plan of Correction: Staff member is doing training of VA Health and Safety Update 2023 and is advised to observe cleaning and hygiene practices.
