Comments:
A complaint inspection was initiated on 7/25/23 and concluded on 7/25/23. There were 79 children present with 11 staff supervising. The inspector reviewed compliance in the areas of staffing and supervision, and programming. The inspection started at 12:15pm and concluded at 1:30pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-350-B-1
Complaint related: Yes
Description: Based on observation and discussions with staff the center failed to ensure that the required infant ratio was met.

Evidence:
On 7/25/23 at 12:15pm the inspector observed that there were 10 infants, aged 2.5 months thru 16 months, with 2 staff.
The required ratio is 1 staff to 4 children.
Staff interviews confirmed that the ratio was not met as the 3rd staff member left for lunch break and the replacement staff member had not yet arrived in the room.
The 3rd staff member arrived approximately 10 minutes after initial observation of the room.
Staff interviews confirmed that this has occurred numerous times a week for the last month.

Plan of Correction: The ratio was corrected within 10 minutes. Staff lunch break schedules will be amended to ensure there is ample time for each staff member to arrive for coverage prior to the other staff leaving for breaks. Staff will also be reminded that they may not leave for their break without coverage actually being in the classroom.

Description: Based on staff interviews the center failed to ensure that children are not punished for toileting accidents.

Evidence:
Staff interviews confirmed that while staff #1 and #2 worked together in the classroom they had been witnessed to place children in time-out and yell at them, on numerous occasions, for toileting accidents within the pre-k classroom.

Plan of Correction: Administration was unaware of this and will immediately take administrative action today. All staff will be reminded that we do not ever yell at the children, nor place children in time-out for toileting accidents.
