Comments:

An investigation was initiated from 10:30 am - 12:45 pm to investigate a complaint regarding behavioral guidance and forbidden actions. Interviews were conducted, video footage, documentation, child records, and staff records were reviewed. The investigation regarding behavioral guidance and forbidden actions is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description:	Based on review of two staff records it was determined that not all records contained documentation of a completed central registry check within the first 30 days of employment. Description:	Based on review of video footage it was determined that behavioral guidance was not always constructive in nature. Evidence: 1.) In the toddler room on 7/20/2023 at 7:40 child B (age 22 months) is seen taking off their shoes. Staff A is seen grabbing child B by the right arm and roughly places the child on the ground. Staff A then places child B's shoes on their feet while subsequently grabbing the child roughly by both arms and lifts the child up off of the ground. Staff A is seen putting their face close to the child's face in an intimidating manner and speaks something to the child that was unable to be determined. 2.) In the toddler room on 7/20/2023 at 3:50 pm child B (age 22 months) is seen playing in the kitchen center of the toddler classroom. Child B moves towards another child and retrieves a toy. Staff B then roughly pulls child B back and forth by the shirt and roughly retrieves the toy from child B's hand. 3.) In the toddler room on 7/20/2023 at 3:56 pm child A (age 21 months) is seen crying and kicking their feet towards child C (age 16 months). Staff B intervenes by picking up child A and roughly places them on the floor away from child C. Staff B does not attempt to console or provide any positive guidance techniques with child A.

Description:	Based on review of video footage it was determined that staff engaged in an action that is forbidden. Evidence: 1.) In the toddler room on 7/20/2023 at 7:40 am Staff A is seen grabbing child B (Age 22 Months) by the right arm and roughly places the child on the ground. Staff A then places child B's shoes on their feet while subsequently grabbing the child roughly by both arms and lifts the child up off of the ground. Staff A then places their face close to the child's face and says something to the child which was unable to be determined. 2.) In the toddler room on 7/20/2023 at 3:50 pm staff B is seen roughly pulling and pushing child B (Age 22 months) by the shirt and retrieves a toy out of the child's hand. 3.) In the toddler room on 7/20/2023 at 3:56 pm child A (age 21 months) is seen kicking their feet towards child C (age 16 months). Staff B picks up child A and roughly places them on the floor away from child C.

Description:	Based on review of video footage it was determined that not all procedures for handwashing were followed. Evidence: 1.) On 7/20/2023 at 7:40 am staff A is seen diapering one child. Staff A failed to wash their hands after completing the diaper change. 2.) On 7/20/2023 between 3:30 pm - 4:00 pm staff B was observed diapering three children. Staff B failed to wash their hands between the three diaper changes.

Staff A failed to clean and sanitize the diaper surface after completing the diaper change. 2.) On 7/20/2023 between 3:30 pm - 4:00 pm staff B was observed diapering three children. Staff B failed to clean and sanitize the diaper surface between the three diaper changes.
